Kamek has a powerful and unique effect that transforms racers into another character, some of which are then unlocked in your Mario Kart World roster.
As such, being transformed by Kamek is very desirable, and though it mostly boils down to luck, there’s a few best practices you can do to increase your chances of seeing this take place.

How to get the Kamek item and be transformed in Mario Kart World
Kamek in Mario Kart World appears alongside other items within item boxes on the track during a race.
In our experience, like some other special items such as the Hammer or Ice Flower, Kamek is rare, and will likely appear just once or twice per Grand Prix — or possibly not at all.
In terms It terms of item distribution, we can verify it can drop as high as third place on the track, and in our experience really appears in either first or towards the very back of the pack.
Either way, if you want to get a Kamek transformation for the purposes of unlocking characters, know you have to be transformed by another player and not have the item yourself, as using the item sees Kamek fly ahead and only inflicts the status effect on other racers in front of you.
This means it pays be towards the front of the grid as often as possible to improve your chances of being transformed.

However, whether the transformations are random — or if there’s a higher chance (or the only chance) of a character transformation appearing on a specific course or region — has yet to be fully confirmed at the time of writing. This thread on the Mario Kart subreddit seems to suggest some characters are more likely to appear on some tracks than others — such as the ghost Peepa in Boo Chima — but isn’t something we’ve been able to fully verify.
You should also know you can be transformed into a character from the starting roster — meaning when you’ve finally managed to get Kamek to appear, it might for a character you already have.
Ultimately, know that not only is Kamek rare, but it’s never guaranteed to transform you into a character you need. As such, until we know the exact specifics of how Kamek works, and / or a reliable farming or unlock method is discovered, it’s recommended you race as best as you can to increase the chances of a Kamek unlock, as sheer playtime and performing well will naturally increase your chances. If you’re struggling, then racing on 50cc or 100cc can help.
So though Kamek is highly random, at least another class of unlockables — costumes — is more reliable to chip away at.
